To be eligible for a Voter Identification Card you need to be 18 years of age on the 1st of January of the year you apply for it. You can submit an application online or in-person by visiting the nearest Booth Level Office. Along with the application you need to submit address proof, age proof, and passport-sized photographs.

Unfortunately, there are no alternative sources for issuing the Voter Identification Card.
You first need to file a report with the police. Once done, you can submit a copy of the report along with an affidavit and address/identity proof at your nearest Booth Level Office. They can then issue a duplicate copy of your Voter Identification Card.
